 Below are two items you will need to print out to get your handpieces or batteries to us. You may click on each item to open them off the site as a PDF, or you can “Right Click” and “Save As” to save them to your computer for printing.  The PDF below are “fillable”, meaning you can use your keyboard and mouse to fill in the information. 

 Print & Fill out the Work Order Form-Place this Inside your Box –BE SURE TO INCLUDE TRACKING NUMBER 

 Please print this out to give us an idea of what is wrong; what you would like done; and if you prefer a free estimate beforehand. We recommend keeping a copy, with tracking number, for your records.

 Simply click on the button below and you will be taken to our USPS Label Center. Follow the directions on that page and simply print out your label​.
